This document is a questionnaire for reference checks regarding a Student Information System (SIS) vendor used by the Washoe County School District. The questionnaire consists of 16 questions that aim to gather feedback and insights from the school district regarding their experience with the SIS vendor and the software. The questions cover various aspects such as the acquisition process of the software, the performance of the system, the quality of vendor training, the implementation team's performance, the vendor's ability to meet schedules and deadlines, the attitude of vendor staff, any problems faced during implementation and their resolution, handling of new releases and upgrades, unexpected surprises during and after implementation, major benefits and limitations of the system, vendor responsiveness to support and maintenance problems, hidden costs during and after implementation, customization issues, and reflections on what could have been done differently in the implementation process. The questionnaire also includes a question asking if the school district would consider using the software and vendor again if they had to start the implementation process over.
