This document outlines the requirements and guidelines for job/project quotations, invoices, remittance information, asbestos documentation, staffing, maintaining services, scheduling of work, and existing conditions for contractors working with the County of DuPage in Illinois. The contractor is expected to provide detailed cost estimates for each job, and job releases may be approved by the County or bid independently. The County will provide floor plans and may provide sketches or diagrams depending on the job scope. The contractor may need to prepare drawings and details for review before starting the work. As installed drawings must be submitted upon completion. The contractor must submit detailed written quotations with a "Not to Exceed" price for each project, based on unit proposal prices. Subcontractors and rental equipment will be billed at cost plus 5%. All work and materials must conform to building codes and local requirements. Any changes or deviations from the specifications must be communicated to the County. Materials must be approved by the County. Invoices must be submitted with required information and documentation, including contract number, description of services, labor and material extensions, work tickets, supplier invoices, subcontractor invoices, equipment rental invoices, and certified payroll. Supporting documentation must be attached for proper cost allocation. Hourly rates must include overhead and profit, and additional charges for tools and supplies are not allowed. Asbestos documentation must be provided, including certification, respirator clearance authorization, fit test documentation, and verification of reading the County's asbestos plan. The contractor must retain the same project coordinators and installation foremen throughout the project and keep adequately staffed crews. Routine and call-out service visits must be checked in with maintenance personnel. The contractor must have employed a minimum of five full-time experienced workers or completed remodeling projects for the County. A superintendent and field foremen must be assigned with contact information. Employees must have a minimum of five years of experience and experience in similar-sized facilities. Subcontractors must be approved by the County. Existing services cannot be interrupted during working hours, and temporary services may be required. Safety measures must be provided, and work must be coordinated with the County. Work schedules must be established in cooperation with the County and other trades. Information and shop drawings must be provided to other trades. Existing ceiling tiles may need to be removed and installed as required. Before bidding work, contractors must submit a Maintenance and Operations Plan and coordinate with the designated person regarding asbestos.
