POL - Card Holder User Agreement

The document is a user agreement for the Purchasing Card program of the City of Hollywood, Florida. The cardholder is explicitly prohibited from using the Purchasing Card for personal purchases, whether for themselves or others. Any intentional use of the card for personal gain may result in disciplinary actions, including termination of employment. The cardholder is required to follow the established procedures for using the Purchasing Card and to comply with all City of Hollywood purchasing requirements. Failure to do so may result in the revocation of their card privileges or other disciplinary actions. The cardholder acknowledges that they have received a copy of the Purchasing Card Manual and understand the requirements for using the card. If the cardholder violates the terms of the agreement and uses the Purchasing Card for personal use or gain, they are obligated to reimburse the City of Hollywood for all charges and any related fees for the collection of those charges. Additionally, the cardholder agrees to process all transactions within two working days of receipt by either approving or disapproving for payment and forwarding to General Accounting for payment, or following the dispute procedure. The agreement requires the cardholder and the Program Director to sign and date the document. It also indicates that the document should be distributed to the cardholder, the approver(s) for the cardholder, the Agency Purchasing Card Administrator, and the Program Purchasing Card Administrator.
