Legal Compliance: The plan should comply with all applicable Florida and Federal Statutes regarding insurance/self-insurance of group benefits, PPOs, POS, MOS, and assure future compliance. 8. Prescription Benefit: The plan should provide a multi-tier copay schedule with a discounted mail-in option. 9. Dental Plan: The plan should provide a multi-tier benefit (preventive, basic, and extended), easy claims handling, and reasonable administrative/premium costs. 10. Employee Assistance Program: The plan should include local face-to-face access to services on an urgent basis, provide a 24/7 crisis line, at least 3 visits per employee per problem at no cost, and a reasonable per employee fee. The plan should also accept employer referrals for substance abuse or other workplace behavioral problems. 11. Group Life Insurance & ADD: The provider should have at least an A+ Best rating. The cost per thousand of insurance should be reasonable, and dependents and retirees should have access to the program. 12. Cafeteria Plan: The plan should provide for premium-only, dependent care, and unreimbursed medical expense payment. The fees should be reasonable, and employee access to the administrator should be convenient. Employee education should also be provided. 13. Classification: The proposal should meet the requirements for being a certified minority/women-owned business. 14. Fees: Proposed fees, rates, and service levels will be compared between like proposals. 15. Presentation: The use of PowerPoint, demographics, graphic illustrations, etc. will be considered in the evaluation. The document provides a scoring system for each criterion, with a weight assigned to each criterion. The total score will be calculated based on the scores given for each criterion.
