This document provides instructions for the submission of proposals in response to RFP number HSS 09-026. The proposals must be submitted in both electronic and printed formats. The electronic submission should include two original CDs labeled as "Original" and two CD copies labeled as "Copy." Additionally, any confidential financial or audit information related to the company should be copied separately to three additional CDs labeled as "Corporate Confidential information." All CD files should be in PDF and Microsoft Word and Excel formats. The printed submission should include eight copies with clearly identified sections for the technical proposal and the business proposal. The proposals must clearly indicate that they are in response to RFP number HSS 09-026. The proposals should be delivered to Ms. Sandra Skelley at the Department of Health and Social Services, Division of Management Services, Procurement Branch. The closing date for the submission of proposals is September 4, 2009, at 11:00 A.M. Proposals must be delivered or sent by certified or registered mail, and no late proposals will be accepted. The notification of the award will be made in writing to all proposers by approximately September 14, 2009. Any questions regarding the RFP should be directed to Ms. Darlene Plummer. All contacts with personnel of the Division of Substance Abuse and Mental Health and other agencies in the Department of Health and Social Services must be cleared through the agency contact. All proposals become the property of the State of Delaware and will not be returned to the proposer. Proposers should mark any proprietary information clearly. The contents of the proposal will be considered binding on the proposer and subject to subsequent contract confirmation if selected. The proposal and the RFP will be incorporated into the final contract. Pre-contract costs and the contractor's equipment are the responsibility of the proposer. The Department reserves the right to reject or accept any bid based on funding limitations. The contract may be terminated if the contractor fails to carry out the provisions or make substantial progress under the terms specified. Clients may be required to pay for services received if financially able to do so. If a proposing firm wishes to request a debriefing, they must submit a formal letter within 10 days after receipt of the outcome of the review and evaluation process. The proposal should be organized according to the format described in the instructions and should include a cover letter, a completed checklist, and consecutively numbered pages. Each proposer is required to submit the Technical Proposal and Business Proposal as separate sections, with all pages numbered consecutively.
