The document consists of two exhibits: Exhibit A, which is an Offer Agreement, and Exhibit B, which is a Questionnaire. Exhibit A is a solicitation agreement where the undersigned offers to provide materials or services in compliance with all terms, conditions, and specifications defined or referenced in the solicitation. The unit pricing includes all costs necessary for the provision of the items, and no additional payment will be made. Conditional offers that modify the solicitation requirements may not be evaluated. The submission of a signed offer agreement constitutes a firm offer, and upon the issuance of a contract, a binding agreement is formed. The resulting contract incorporates all requirements set forth in the solicitation, solicitation instructions, standard terms and conditions, and other listed documents. Exhibit B is a questionnaire that will be used by an evaluation committee to assign points to each proposal submitted. The evaluation criteria include customer focus, competitiveness/continuous improvement, and training. The questionnaire asks the offeror to demonstrate their expertise and focus on customer satisfaction, provide information and documentation related to customer focus, demonstrate competitiveness and commitment to continuous improvement, present technical capabilities in electronic banking services, demonstrate commitment to training, compare staff training to competitors in the Tucson area, and provide a current and future training plan for the account. Zero points will be given for a criterion if there is no documentation to review.
