It states that the business has and adheres to a written policy that provides at least five days of regular pay for actual jury service for full-time employees who are also California residents, or the business will have and adhere to such a policy prior to the award of the contract. The form also includes a declaration under penalty of perjury that the information provided is true and correct, and space for the business's name, title, signature, and date. The document also includes standard terms and conditions related to pricing, assignments by the contractor, and the Safely Surrendered Baby Law. The Safely Surrendered Baby Law requires the contractor to notify and provide information to its employees regarding this law. However, this information is not directly related to the Jury Service Program.
