This document is a contract between the State of Florida, Department of Health, referred to as the department, and a provider. The contract outlines the agreement between the two parties regarding the provision of services. The provider agrees to comply with the conditions specified in Attachment I and to submit deliverables, reports, and drafts for review and acceptance by the contract manager before payment. The provider also agrees to submit bills for fees and expenses in detail, including travel expenses, and to allow public access to all documents related to the contract. The contract is governed by the laws of the State of Florida and, if applicable, federal laws and regulations. The provider must comply with federal regulations if the contract involves federal funds. The contract also includes provisions regarding the prohibition of employing unauthorized aliens, compliance with the Pro-Children Act of 1994, and compliance with the Health Insurance Portability Accountability Act (HIPAA). The provider is required to maintain proper records and documents in accordance with accounting procedures and practices and retain them for a period of six years after the termination of the contract. The department and authorized personnel have the right to inspect and audit the provider's records. The provider must also cooperate with the department in duplicating and transferring records upon completion or termination of the contract. The contract includes requirements for financial and compliance audits and the documentation and reporting of expenditures. Additionally, the provider is required to submit a report within 45 days of the end of the contract, detailing the direct results of services provided through the funding of this contract to the Department.
