It asks if the officer is receiving or likely to receive taxable income from the filer, if the filer is receiving or likely to receive taxable income from the officer, if the filer is affiliated with a corporation or business entity that the officer serves as an officer or director or holds an ownership of 10 percent or more, and to describe each affiliation or business relationship. The sixth section requires the signature and date of the person doing business with the governmental entity. The document also includes information about a bid (#12124-09) for maintenance road microsurfacing, but there are no questions associated with this bid.
