All blank spaces must be correctly filled in, and unit prices or lump sum prices must be stated for each item. Bidders must acknowledge receipt of any addendum issued. - All prices quoted by the bidder must be F.O.B. delivered and include all freight costs to the specified delivery location(s). - Bids must be submitted for all items unless otherwise directed. Unit prices must be extended and the total amount of the bid indicated on the proposal pricing page. - Bids will be compared based on the number of units stated in the Bid Pricing Section, and payment will be based on the actual number of units installed in the completed work. - Bidders are warned against making erasures, alterations, omissions, or additions not called for in the bid. All blanks must be filled in, and "N/A" or "None" should be used where applicable. - The bid must be executed by the President of a corporation, all partners of a partnership, or the owner of a sole proprietorship. Evidence of authority may be required if the bid is executed by someone other than the President or all partners. - Bids must be submitted in sealed envelopes with the bidder's name and address on the upper left corner and the attached label filled in and pasted on the lower left corner. Additionally, bids mailed via express mail must follow specific instructions provided in the document.
