Invoices should be sent to the Govalle Wastewater Treatment Plant and should include the description of the item delivered, quantity, unit price, total price, and contract number. Due to security requirements, the contractor is required to fax a "Chemical Delivery Notice" form to the delivery site upon receiving a purchase order and dispatching drivers.
