The offerors also need to complete the Proposal Cost Response Form, which includes the proposed cost for all services, including delivery and pick-up, price protection period, itemized listing of optional cost elements, and differentiation of costs by daily fee, etc. Finally, the offerors need to provide insurance requirements, including a certificate of insurance or a copy of insurance declaration pages, as evidence of their ability to meet the insurance requirements listed in the RFP. They also need to provide a letter from an insurance agent or other appropriate insuring authority documenting their willingness and ability to endorse their insurance policies making the County an additional insured.
