This document is a form that businesses need to fill out in relation to the Jury Service Program. The form has two parts - Part I and Part II. In Part I, the business can claim that the Jury Service Program is not applicable to their business. They can make this claim if their business does not meet the definition of a "Contractor" as defined in the Program. This means that their business has not received an aggregate sum of $50,000 or more in any 12-month period under one or more County contracts or subcontracts. However, if their revenues from the County exceed $50,000 in any 12-month period, they will lose this exception and must comply with the Program. Additionally, if their business is a small business, as defined in the Program, with ten or fewer employees and annual gross revenues of $500,000 or less, they can claim an exemption. But if the number of employees and gross annual revenues exceed these limits, they must comply with the Program. The document also provides definitions for "Dominant in its field of operation" and "Affiliate or subsidiary of a business dominant in its field of operation". Lastly, if the business is subject to a Collective Bargaining Agreement that supersedes the provisions of the Program, they can claim an exemption. In Part II, the business can certify their compliance with the Program by stating that they have a written policy that provides at least five days of regular pay for actual jury service for full-time employees who are California residents. They must also declare that the information provided is true and correct. The document also includes standard terms and conditions related to payments for goods and services provided to the County, and restrictions on assigning rights or delegating duties under the Agreement. Additionally, there is a mention of the Safely Surrendered Baby Law, which requires the Contractor to notify and provide information about the law to its employees.
