Once all donated leave is used, a copy of the form is sent to the donating employee. Unused leave donation forms should be returned to the donating employee once the receiving employee returns to work. The document also provides conditions and instructions for the leave donation process. Donated leave can only be used for authorized incapacitating illnesses or disabilities. Donations must be voluntary and made from accrued leave. Donated leave cannot be recovered once used, unless reimbursed by Worker's Compensation. Leave must be donated in one-hour increments, and any annual leave donated will be converted to sick leave. Donated leave cannot be used retroactively or to reduce a negative leave balance. The instructions outline the steps for completing the form and the responsibilities of the donating employee, the Department/Agency Head of the receiving employee, and the Timekeeper. It also emphasizes the need for confidentiality regarding the donating employee's identity unless they choose to make their donation known personally.
