The bidder certifies that they are not currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in district, state, or federal procurements or contracts. They also confirm that they are not listed in the "Lists of Parties Excluded from Federal Procurement or Non-Procurement Program" issued by the General Services Administration. The form concludes with a section for the authorized agent of the company to provide their name, signature, and the company name. The agent certifies that the information regarding felony convictions has been reviewed and is true to the best of their knowledge. They also confirm their agreement to comply with the relevant sections of the Texas Education Code and federal procurement regulations.
