The Contractors Wage Certification form is a certification form that must be completed by contractors who have been awarded a contract with a value of $100,000 or more. The form requires the contractor to certify that they and all their subcontractors will pay all workers on the project the wages listed in the current Department of Labor prevailing wage rates, as required for the project. The form must be signed and notarized, and a copy must be sent to the Labor Department's Wage & Workplace Standards Division.
